CHYS MA Program Forms
Procedures and Forms for the CHYS Graduate Program
At the beginning of each semester you must complete the graduate registration form. This form is to be signed by your thesis advisor and the CHYS Graduate Program Director. Once signed, you must submit this form directly to the Office of Graduate Studies.
Progress Report Form
A progress report is to be completed at the end of each semester that you are registered in the program (3 times per year). The progress report should be completed in coordination with your thesis advisor. Please have your advisor sign the completed form and return the form to the Graduate Program Director by the last day of each semester.
Thesis Proposal Form
At some time close to the end of your first year within the program you should arrange to meet with your thesis committee members to discuss and formally approve their thesis proposal. The nature of the thesis proposal meeting will vary according to the agreement made between you and your thesis advisor. In some cases students may be asked to make a formal presentation of their thesis work to their thesis committee, while in other cases students may simply have a discussion or meeting with their thesis committee. Once your thesis committee has approved your thesis proposal, the thesis proposal form should be signed by each member of your committee and submitted to the CHYS Graduate Program Director. We encourage full-time students to have their thesis proposal approved by no later than the end of their third full time semester (typically by the end of the summer of Year 1 in the program).
First Draft Form (Reduced Tuition Policy)
Once you have completed the first draft of your thesis you may qualify for reduced tuition for the final semester of your program. Please complete the First Draft Form, have it signed by your thesis advisor and submit it to the CHYS Graduate Program Director.
Application to Graduate
Your must apply to graduate at the end of your program (even if you do not intend to attend the convocation ceremony). Brock has 2 graduation ceremonies, in June and October of each year. You must apply to graduate by February 1st if you wish to graduate in June, and you must apply by July 1 if you wish to graduate in October. There is a fee or $35 associated with this application. Applications must be signed by the student and submitted directly to the Office of Graduate Studies.
Completed Thesis Approval Form
Once you have completed the final draft of your thesis you should arrange to have all of your thesis committee members read and approve your thesis. The thesis approval form should be signed by all members of your thesis committee. Once this form is signed by all thesis committee members, you should submit the following to the CHYS Graduate Program Director: 1 Thesis Approval Form signed by all members of your thesis committee
- 5 unbound copies of your thesis
- 3 names and contact information for possible external examiners
- At least 5 possible defence dates that are available to each of your thesis committee members
Your thesis defence will be scheduled approximately 4 – 6 weeks after you have submitted all of the above.
Thesis Defence Procedures
Thesis defence procedures vary across departments and universities. The following document will provide you with information about the thesis defence procedures followed by the CHYS department. Please contact the CHYS Graduate Program Director if you have any questions about your thesis defence.
Title Page Template
Graduate theses completed at Brock University must include a standard title page. Please use this template to prepare the title page for your thesis.
Library and Archives Canada Form
Once your thesis is completed and bound a copy is kept in the Library of Canada Archives. This form must be completed in order to release a copy of your thesis to the Library of Canada. Please submit 2 completed copies to the CHYS Graduate Program Director when you hand in the final copies of your thesis.
Partial Copyright Licence
This form is required after May 1, 2009. This form allows the Brock University Lirary the right to lend the thesis to users of the Brock University Library, to make partial or single copies of the thesis for users of the Library or in response to a request from the library of any other university or educational institution. It also allows for digital copies for use in its circulating collection.
