OSAP paper application
We strongly encourage students to use the online OSAP application process. It is the fastest way to get an OSAP assessment, and includes built-in error checking. If you are unable to use the online application because of special circumsntances, you may apply via paper application using the form below.

OSAP Processing Forms
The following forms allow you to make changes or updates to your OSAP application. Completed forms must be submitted to the Student Awards and Financial Aid office no later than 8 weeks prior to the end of your study term. Forms are normally processed within 2 weeks of receipt.
Status Change
Use the appropriate forms below if you are changing your student status (married, single dependent, single independent, or sole-support parent). A student income update must accompany all status change forms. Be sure to include all other requried supporting documentation.

Income Change
Use this form to update your income information. Note that you may also update your income when completing your confirmation of enrolment when picking up your first OSAP installment.
Income Appeals
Use these forms to request a review of your OSAP file based on exceptional circumstances regarding your income, your parent's income, or your spouse's income.
Adjust Income Contribution due to Lack of Employment
Request for Review to consider adjustments to the student’s expected income contributions in the pre-study period and from a student’s spouse in the pre-study and study period (if applicable).
Assessment on Parent's Estimated Income
Form student uses to request that his or her aid be calculated using parent's estimated 2012 income rather than actual 2011 income

Asset Appeals
Use the form listed below to request a review on your vehicle exemption level. If you have other assets on your application which may reduce your assessment, you may contact our office to discuss your cirucmstances. In most cases, assets as declared on the OSAP application are considered as a resource to contribute to your educational costs.
Vehicle Exemption Level
Request for Review to consider a reduction to the expected contribution from the vehicles(s) that you (and your spouse, if applicable) own or lease.
Vehicle Appraisal Record
Use this form if one or more vehicles owned or leased by you are overvalued on your application.
Expense Appeals
Use these forms to request a review of your OSAP file based on exceptional expenses that you or your family have incurred during the pre-study or study period. Please contact our office for exceptional expenses not listed below (examples include legal costs, uninsured medical costs, essential home repairs, etc.).
Adjust Living Allowance
Use this form if you are:
- A single, independent student who lives with family but pays room and board OR
- A single, dependent student who lives away from home during the pre-study period in order to work full-time, OR
- A single, dependent student who does not live with family during the school year, even though the family home is within 40km of Brock, because it takes more than one hour by public transit, or because public transit is not available for your commute, OR
- A single, dependent student who does not live with family during the school year because the family home is overcrowded.
Adjust Local Travel
Use this form if you are commuting more than 40km to Brock, or if it takes more than one hour by public transit, or if public transit is not available for your commute.
Other OSAP forms
Request for a Temporary OSAP Password
Request a temporary password when you have been locked out of the OSAP website.
Note: there is a faster way to get your OAN number and password. Just drop by the Student Awards and Financial Aid Office with your Social Insurance Card and a valid, government issued photo ID. You are also welcome to drop by any other financial aid office at another post-secondary institution that administers the OSAP program to retrieve your OAN number.
Download a Request for Continuation of Interest-Free Status (Schedule 2)
This is for full-time students who will NOT be receiving OSAP loan funding for this current academic year and session. It is used to ensure that your previous loans continue in interest-free status. Students who have previously defaulted on their student loans or who have declared bankruptcy, which included OSAP loans, are not eligible for interest-free status. Please send or drop off this document to the Student Awards and Financial AId Office for processing.
