During this closure, medical staff will be in the office on Thursdays from 9 a.m. - 1 p.m. to retrieve and answer telephone messages and follow up on incoming faxes.

If you require medical information to be sent to yourself or another healthcare provider, please fax written authorization to 905-688-6448.  This authorization must include:

• Your name.
• Your student #.
• The information you would liked faxed.
• To whom you would like the information faxed.
• The fax number you would like the information faxed to.
• Your signature.
• A telephone number we can reach you at should we have any questions.
