Information for Health Care Providers

The Brock University Heart Institute acts as a secondary prevention program for people with diagnosed heart disease. As such, this cardiac rehabilitation service is designed to treat individuals in order to prevent a first or subsequent cardiovascular event. In most cases, these individuals will have experienced at least one of the following:

  • Myocardial Infarction (Heart Attack)
  • Coronary Artery Bypass Surgery
  • Angioplasty
  • Angina
  • Congestive Heart Failure

This program also acts as primary prevention for people at high risk for heart disease. These participants will have one or more of the following risk factors:

  • Hypertension (High Blood Pressure)
  • Diabetes
  • Dyslipidemia
  • Overweight/Obesity

 

If you have a patient that fits the above profile, please download our referral form. Please fill out the referral form and fax it to our office.

A staff member from the Heart Institute will then contact the referred patient directly to set up an appointment as soon as possible.
