Add / Withdrawl Form  

For students adding or dropping a Co-op work term after the online registration deadline has passed. In addition students must submit either the Request for a Backdated Withdrawal Form (to drop a Co-op work term) or the  Request For a Retroactive Registration Form (to add a Co-op work term).

For students wishing to drop a Co-op work term after the online deadline has passed.

 

Change of Major Form

For students wishing to add or remove the Co-op option from their degree program.
