Information for Health Care Providers
The Brock University Heart Institute acts as a secondary prevention program for people with diagnosed heart disease. As such, this cardiac rehabilitation service is designed to treat individuals in order to prevent a first or subsequent cardiovascular event. In most cases, these individuals will have experienced at least one of the following:
- Myocardial Infarction (Heart Attack)
- Coronary Artery Bypass Surgery
- Angioplasty
- Angina
- Congestive Heart Failure
This program also acts as primary prevention for people at high risk for heart disease. These participants will have one or more of the following risk factors:
- Hypertension (High Blood Pressure)
- Diabetes
- Dyslipidemia
- Overweight/Obesity

If you have a patient that fits the above profile, please download our referral form. Please fill out the referral form and fax it to our office. A staff member from the Heart Institute will then contact the referred patient directly to set up an appointment as soon as possible. |
