How do you make a Doctors Appointment?
For all health concerns please book an appointment by calling 905-688-5550 ext. 3243 or stop by Health Services during operating hours.
For your FIRST visit each school year you will be required to complete a Demographic and Policy Form and a Confidentiality and Medical History Form. Please complete these forms and bring them with you to your appointment. Remember, you need to complete this form only ONCE per school year.
