Student Performance Evaluation

Please review the Student Performance Evaluation form at the begining of your work term in order to become familiar with the criteria your performance will be measured against. Student Performance Evaluations are typically completed at the end of the work term, however you should seek frequent feedback related to your performance throughout the assignment.  Please ensure that the Student Performance Evaluation is provided to your employer and submitted to the Co-op Programs Office by the last day of the co-op work term.  The evaluation becomes part of your student record and is required to ensure proper recognition and transcript notations are made. Employers may choose to use their own performance management tools; this is acceptable but should be submitted to the Co-op Programs Office for review.  You can scan and upload your evaluation to CareerZone in the same manner as described above in Work Term Report.

Work Site Visit

At some point during your work term, a representative from Co-op Programs Office will arrange to meet with you and your immediate supervisor to discuss your work term performance, additional work term assignments, and to find out more about the organization and potential co-op opportunities. The time for this visit will be arranged between yourself, your supervisor and a Co-op representative and will last approximately 1 hour. Most work site visits take place in person, however in some cases meetings may be arranged via phone. It is important that you prepare for the work site visit in advance as this will ensure any outstanding questions and concerns are resolved. Remember, do not wait until the work site visit to raise any serious problems or issues regarding your work term. Contact the Co-op Programs Office immediately if you feel it necessary to do so.
